Access Aged Care is redefining how General Practitioners practise within Residential Aged Care Homes, creating a more sustainable and clinically supported way to practise medicine in aged care. Backed by the national network of Access Telehealth, our multidisciplinary model enables GPs to focus on timely, proactive resident care while working alongside dedicated nurses, telehealth specialists and streamlined operational systems. Designed for clinicians who want to continue practising in aged care long‑term without the unsustainable pressures traditionally associated with the sector, this model supports proactive intervention, collaborative care and better health outcomes for older Australians.
Our integrated model of care has contributed to measurable reductions in emergency department presentations, unplanned hospitalisations and polypharmacy within partnered Residential Aged Care Homes nationally. Through our national network of general practitioners, nurses and specialists we support Residential Aged Care Homes and regional communities nationwide while improving access to high‑quality healthcare for older Australians.
This opportunity offers weekday hours only, with no after‑hours, weekends or on‑call requirements. With dedicated after‑hours support in place you can switch off between sessions and protect your personal time.
You will work with a consistent cohort of residents, allowing you to deliver proactive, relationship‑based care and build meaningful long‑term clinical relationships. An Access Aged Care Nurse supports your rounds, residents and follow‑up care, helping coordinate care plans, investigations and communication with homes so your sessions run smoothly and efficiently.
Supported by telehealth geriatricians, physicians and specialists, you will have trusted backup for complex clinical decision‑making and preventative resident care, helping address issues early before they escalate into avoidable deterioration or hospital presentations.
Our integrated model includes shared clinical records, streamlined systems and dedicated practice support, reducing paperwork, coordination and operational burden so you can focus on what matters most – less paperwork, better medicine.

Requirements
- Current AHPRA Specialist Registration as a General Practitioner.
- RACGP or ACRRM Fellowship required (or nearing completion).
- Availability of 1 day per week initially, with opportunity to increase.
- Comfortable with technology, including system‑based reporting and common clinical software.
- A personal laptop is required to perform the duties of this role.
