Booraka Danjoo Assertive Outreach Workers

Working in partnership with Karlup Aboriginal Corporation and Indigo Junction, the Assertive Outreach Worker will deliver culturally safe, trauma-informed, and person-centred outreach services to people experiencing homelessness and vulnerable community members across Perth’s North East Metropolitan Corridor.
The Assertive Outreach Worker will provide triage, assessment, advocacy, referrals, and short-term case management support to individuals and families experiencing homelessness and crisis. This role plays a key part in strengthening collaborative responses to homelessness while fostering positive outcomes for Aboriginal and Torres Strait Islander peoples and the broader community.
About the Role
Working within a Trauma-Informed Care framework, you will:
- Provide triage, support, and short-term casework to individuals and families accessing the service.
- Deliver informal counselling, advocacy, referrals, and practical support to clients experiencing crisis and homelessness.
- Support people homeless to access immediate crisis support, facilities, and essential services.
- Conduct vulnerability assessments and support planning with individuals and families experiencing homelessness.
- Assist with the delivery of individual and group life-skills programs.
- Support clients to access housing, health, alcohol and other drug services, community resources, and government supports.
- Build positive relationships with community organisations, government agencies, and key stakeholders to support client outcomes.
- Maintain accurate client records, data management, case notes, and reporting requirements.
- Promote a culture of empowerment, self-determination, and culturally safe practice.
- Work collaboratively with internal teams and external service providers to deliver integrated support responses.
About You
To be successful in this role, you will have:
- A tertiary qualification in Human Services, Social Work, Community Services, Psychology, or a related field, with a minimum of three years’ relevant experience, or demonstrated equivalent experience.
- A sound understanding of the principles of Trauma-Informed Care.
- Demonstrated experience and competence working with people in crisis and experiencing complex needs.
- Experience working within a case management framework, with a strong understanding of professional boundaries.
- Demonstrated experience providing advocacy, referrals, and practical support.
- Knowledge of community, housing, and government support services.
- An understanding of the cultural and social factors contributing to homelessness and crisis.
- Experience supporting people affected by homelessness, alcohol and other drug issues, mental health challenges, and other complex needs.
- A strong commitment to empowerment, self-determination, and person-centred practice.
- Cultural competence and experience working with people from diverse backgrounds, including Aboriginal and Torres Strait Islander communities.
- High-level interpersonal, communication, negotiation, and advocacy skills.
- Strong written communication skills, including case noting and record keeping.
- Excellent organisational, time management, and self-management skills.
- Sound computer skills, including experience using databases, spreadsheets, and Microsoft Office applications.
- Current C Class Driver’s Licence.
- Current National Police Clearance.
- Current Working with Children Check.
- Current First Aid Certificate (or willingness to obtain).
- Ability to travel throughout the Perth Metropolitan North East Corridor as required.
